LPN Med Care Manager

The LPN Medication Nurse provides licensed nursing support in senior care settings by administering medications, monitoring residents’ health, and ensuring accurate clinical documentation.

• Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration.
• Accurately document in MAR/TAR in real-time and support care plan updates.
• Monitor symptoms, report adverse reactions or refusals, and observe physical, emotional, or cognitive changes.
• Conduct Service & Health Updates (SHUs), transcribe orders, and maintain medication carts with proper narcotic counts.
• Uphold compliance standards and prevent medication errors to ensure resident safety.

• CPR and First Aid certification.
• Strong assessment, observation, and documentation skills.
• Experience in senior care or medication administration preferred.
