About the Role

The Admission Nurse plays a critical role in the long‑term care setting by conducting comprehensive assessments of new residents to ensure their health needs are accurately identified and addressed from the outset. This position involves collaborating closely with interdisciplinary teams to develop individualized care plans that promote resident well‑being and safety. The nurse will serve as a key liaison between residents, families, and healthcare providers, facilitating smooth transitions into the facility.

Attention to detail and thorough documentation are essential to maintain compliance with healthcare regulations and quality standards. Ultimately, the Admission Nurse ensures that each resident receives personalized, compassionate care tailored to their unique medical and psychosocial needs.

Schedule:
Part Time Thursday & Friday 10a-10p

Minimum Qualifications
  • Valid LVN or RN license in the state of employment.
  • Minimum of 1 year experience in long‑term care or a related healthcare setting.
  • Basic Life Support (BLS) certification.
  • Strong knowledge of nursing principles, patient assessment, and care planning.
  • Ability to communicate effectively with residents, families, and healthcare team members.
Preferred Qualifications
  • Experience with electronic health record (EHR) systems specific to long‑term care.
  • Additional certifications such as Certified Dementia Practitioner (CDP) or Geriatric Nursing Certification.
  • Familiarity with state and federal regulations governing long‑term care facilities.
  • Experience in patient education and counseling.
  • Demonstrated skills in interdisciplinary team collaboration.
Responsibilities
  • Perform thorough admission assessments including medical history, medication review, and physical evaluations for new residents.
  • Coordinate with physicians, social workers, and other healthcare professionals to develop and implement individualized care plans.
  • Educate residents and their families about facility policies, care procedures, and available support services.
  • Document all admission information accurately and maintain up‑to‑date resident records in compliance with regulatory requirements.
  • Monitor and report any changes in resident condition during the admission process to ensure timely interventions.
  • Assist in the orientation of new residents to the facility environment and routines.
  • Ensure adherence to infection control protocols and safety standards during all admission procedures.
