Hospice RN Case Manager — Patient-Centered Home Care

St. Gabriel's Hospice and Palliative Care is seeking an experienced RN Case Manager to admit and oversee nursing care for patients at their homes.

You will coordinate with the Interdisciplinary Group/Team, patients, families, and referring agencies to ensure comprehensive, compassionate care. Responsibilities include assessing patients, creating and documenting care plans, delivering nursing care with infection control, and participating in QAPI programs.
