Your Role in Our MissionAs part of our interdisciplinary team, you will:
- Assess patients' health needs and develop individualized care plans.
- Coordinate services to ensure smooth transitions across healthcare settings.
- Communicate effectively with patients, families, and healthcare providers.
- Advocate for patients' needs and promote health education.
- Utilize clinical expertise and critical thinking to enhance patient outcomes.

Qualifications to ShineRequired
Education:
- Graduate of an accredited School of Nursing.
- Bachelor's Degree in Science of Nursing.
Required Experience:
- 1 year of experience in an acute care hospital setting.
Required Licensure:
- Current RN License in the state of employment.
Required
Certifications:
- BLS (Basic Life Support) certification at hire date or within 90 days.
Preferred Qualifications:
- Certification in Case Management.
- 2 years of experience in an acute care hospital setting.
- Experience in Care Management or Utilization Management.
