As an RN Care Manager, you will play a vital role in coordinating and advocating for patient care from the moment they are admitted to the hospital until they are safely discharged. Your expertise will ensure smooth transitions and promote quality outcomes, all while embodying Mercy's mission and service standards.

As a key member of our interdisciplinary team, you will:

• Assess patients' health needs and develop individualized care plans.
• Coordinate services to optimize health outcomes and ensure safe transitions post-hospitalization.
• Communicate effectively with patients, families, and healthcare providers.
• Advocate for patients' needs and promote health education.
• Utilize best practices and performance metrics to enhance patient experience and safety.

Education:

• Bachelor's Degree in Nursing (BSN) - required

Licensure:

• Current RN License in the state of employment - required

Experience:

• Minimum of 1 year in an acute care hospital setting - required

Certifications:

• BLS (Basic Life Support) certification at hire or within 90 days - required
• Certification in Case Management - preferred
