Responsibilities:- Provide direct care to residents, including administering medications, dressing wounds, and assisting with daily living activities
- Monitor and report changes in residents' conditions to the nursing team and other healthcare professionals
- Develop and implement individualized care plans in collaboration with the interdisciplinary care team
- Participate in wellness and recreation activities with residents, promoting their physical, emotional, and social well-being
- Maintain accurate and up-to-date records of resident care and services
