The Physical Therapist (PT) provides high‑quality, patient-centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and care team. The PT completes patient assessments, develops and implements individualized care plans, and provides or directs therapeutic care throughout the continuum of care.

• Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
• Assess therapeutic status and participate in the development of the total plan of care.
• Provide purposeful therapeutic care at the patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
• Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• Prioritize caseload, determine daily staffing needs, and coordinate assignments/handoffs.

• Basic Cardiac Life Support (BCLS) within 30 days of employment.
• Licensed Physical Therapist in Texas.
• Bachelor’s degree from an accredited PT program.
• Minimum of 2 years experience in an acute care facility.
• Ability to work 2 weekends per month.
