The Physical Therapist (PT) provides high‑quality, patient‑centered care by managing the complete process of therapeutic care for an assigned group of patients. PTs complete patient assessments, develop individualized care plans, and direct therapy to support patients throughout the continuum of care.

• Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
• Assess and evaluate therapeutic status and participate in developing the total plan of care.
• Deliver purposeful, skilled therapeutic care at patient’s home, using procedures and techniques to produce a change consistent with the physician’s plan of care.
• Instruct patients, family, and caregivers in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• Prioritize caseload, determine daily staffing needs, and coordinate assignments and handoffs.

• Basic Cardiac Life Support must be obtained within 30 days of employment start date.
• Licensed Physical Therapist (LPT).
