Zorgfabriek in Weert zoekt een trajectbegeleider voor jongeren die om verschillende redenen niet thuis kunnen wonen. Je ondersteunt hen in hun ontwikkeling naar zelfstandigheid en maakt plannen op maat voor drie jongeren.
Werken in een warm team met humor is essentieel, waarbij je samenwerkt met andere trajectbegeleiders en woonbegeleiders. De werktijden bestaan uit ochtend-, late en slaapdiensten.
