Zorgfabriek zoekt een trajectbegeleider voor een woongroep nabij Weert. Je verzorgt drie jongeren, maakt op maat plannen, en bewaakt het netwerk van scholen, familie en verwijzers. Je werkt mee aan de uitvoering en houdt het overzicht.
De jongeren hebben uiteenlopende problematieken zoals autisme, ADHD en hechtingsproblematiek. Je blijft kalm bij intensieve situaties en coördineert zorg in een warm team van woonbegeleiders, collega-trajectbegeleiders, een coördinator zorg en een teamleider.
