CHANGE, Inc Family is hiring a Full Time Patient Care Coordinator
Qualifications- Active LPN Licensure in WV and OH
- Excellent organization, critical thinking, communication, interpersonal and multi-tasking skills, excellent documentation, and computer skills.
- Applicant must be able to work well with diverse patients and staff and promote a positive image for CHANGE, Inc.
- Must maintain a valid driver’s license.
- Pass a background check and drug screening.
- Coordinate patient care with a Family Medical Care Provider and assist the medical team with any other duties deemed necessary.
