201133 - WMCH-Skilled Care Certified Nurse Assistant

Provides quality, cost-effective healthcare under the supervision of a registered nurse or licensed practical nurse. Emphasis shall be placed on maximizing the patient’s functional abilities in daily activities; promoting, maintaining, and restoring health, preventing and minimizing the disabilities of acute and chronic illness. Performs duties in a responsible, professional manner while demonstrating respect, dignity, integrity, and compassion. Actions are consistent with hospital policies and current nursing practice standards.

Minimum Qualifications
  • High School diploma or equivalent.
  • Current Certified Nursing Assistant Certification through the West Virginia DHHR.
  • CPR certification.
Experience

Preferred Qualifications
  • Three (3) months experience as a nurse assistant.
Core Duties and Responsibilities
  • Follow established plan of patient care.
  • Assist with admission, transfer, discharge, and transport of all patients.
  • Implement patient safety measures, including but not limited to:
    • Securing patients appropriately for transport.
    • Assisting patients with positioning, transfers, and ambulation.
    • Ensuring call light is within easy reach.
  • Collect and record vital signs as instructed by the RN or LPN.
  • Provide measures to meet patient comfort needs.
  • Collect and prepare specimens according to policy and procedure—including urine, stool, and sputum.
  • Administer care and treatments for patients as instructed by the RN, including but not limited to:
    • Weigh patients.
    • Give enemas.
    • Provide activities of daily living/restorative care.
    • Perform perineal, catheter, and oral hygiene care.
    • Apply hot or cold compresses.
    • Strain urine.
    • Obtain T.P.R. (temperature, pulse, respirations).
    • Obtain intake and output.
    • Ambulate patients.
    • Turn and position patients to maintain proper body alignment.
    • Provide sitz bath, pre‑operative, post‑operative, and post‑mortem care.
    • Apply assistive devices, braces, and equipment.
    • Apply restraint alternatives where applicable.
  • Perform basic routine duties, including but not limited to:
    • Distribute fresh water every shift.
    • Dispose of soiled linen every shift.
    • Maintain a clean and orderly unit.
    • Maintain and stock unit supplies.
    • Practice clean and sterile technique while maintaining body substance isolation.
    • Be aware of the location of equipment and supplies on the nursing unit.
    • Demonstrate knowledge and skill necessary to provide care and education appropriate to patient age (Adult 18‑79, Geriatric 80+).
Food and Nutritional Duties
  • Distribution of snacks and supplements.
  • Maintain fluid restrictions.
  • Assist patients with meals and supplements as needed.
  • Document in the electronic medical record.
  • Provide care for respite and hospice patients.
  • Attend in‑services and other educational programs related to geriatric care.
  • Participate actively in a minimum of three staff meetings annually.
  • Demonstrate proficiency, competence, and current knowledge in patient care.
  • Determine areas of self‑improvement.
  • Support and adhere to hospital, nursing service, and skilled care policies and procedures.
  • Provide restorative patient care that promotes or enhances the patient’s quality of life.
  • Participate in continuous improvement activities.
  • Set short‑term goals to organize work to meet patient needs.
  • Institute emergency measures calmly and effectively.
  • Prepare and submit incident reports.
  • Report safety or risk management issues to director, charge nurse, or appropriate personnel.
  • Support hospital and departmental cost‑containment efforts.
  • Review and initial meeting minutes and postings in the department.
  • Respond to patient call light immediately; notify nurse of patient problems.
  • Report observations, requests, and unusual signs or symptoms to charge nurse or team leader immediately.
  • Keep director informed about patient care issues and departmental activities.
  • Assist with implementation of the activities program as needed.
  • Provide planned individual and group activities or activities of patient’s choice.
  • Ensure patient bedside calendar is updated or replaced.
  • Transport patients to and from activities.
  • Notify activity personnel of pertinent patient care information (e.g., fluid restrictions, isolation).
