Key Responsibilities- Provide care and education appropriate to adult patients (18‑79 years) and geriatric patients (80+ years).
- Assess patient status and report age‑specific needs to the registered nurse.
- Perform wound care, dressing changes, ostomy care, irrigations, and surgical drain checks.
- Maintain pharmacologic treatments, administer medications, and perform peripheral IV access after training.
- Document patient status, abnormal findings, and nursing diagnoses daily.
- Collaborate with the multidisciplinary team to develop and implement individualized plans of care.
- Follow hospital policies, regulations, and long‑term care requirements.
- Current licensure as a Practical Nurse with the WV Board or multi‑state licensure via the Nurse Licensure Compact.
- CPR certification.
- Minimum three months of LPN experience.
- Lift, move, stoop, reach, and position patients and material weighing 40+ lbs. daily.
- Standard clinical environment with very high patient volume and exposure to blood borne pathogens, bodily fluids, toxins, and chemicals.
