Nursing Consultant; NC SSSD

The Senior Select Services Division Nurse Consultant serves as a nursing consultant for assigned clients at an assigned location. Within this framework, provides a full range of indirect nursing services designed to enhance the quality of independent living of clients. These services include, but are not limited to, conducting necessary assessments, developing client specific medical plans of care, and provide follow along services for persons who are at risk of nursing home placement.

Essential job duties and responsibilities may include some or all of the following essential job functions.

• Demonstrate regular, timely attendance as evidenced by reporting to work/meetings when expected and utilizing PTO per Agency policy.
  Actively participate in activities with other members of the care team (including Agency and non-agency members) and effectively implement recommendations for the purpose of ensuring continuity of care.
• Maintain confidentiality of client, personnel and Agency information.
  Conduct nursing assessments of clients and document findings in an accurate and timely manner as defined by regulatory and Agency policy.
• Complete all necessary documentation to ensure continuity of care for all assigned clients.
• Train staff and/or providers on client specific medical issues.
• Document the provision of nursing services and maintain client charts/ records, using the established recording system and in compliance with applicable policies.
• Conduct person centered periodic monitoring of clients by telephone or face-to- face visits.
• Identify need for and obtain durable medical equipment
• Identify and define problems, collect data, establish facts and draw conclusions, utilizing prudent nursing judgment in both emergent and non-emergent situations.
• Recognize abnormal objective findings based on physical assessments, report as needed to physician, and complete/document all follow-up as needed.
• Demonstrate commitment to CCIL by taking responsibility for job functions.
• Assist with the promotion, coordination and delivery of other Agency services.
• Read, analyze, and interpret government regulations as evidenced by adherence to those regulations.
• Assure all paperwork/documentation/forms necessary to bill for appropriate services to clients is completed accurately and in a time frame defined by Agency policy.
• Assure all documentation necessary to ensure continuity of client care is completed accurately and in a time frame defined by Agency policy.
• Recognize situations that must be reported to Adult Protective Services or Child Protective Services and inform the appropriate services of the same.
• Effectively communicate as evidenced by conveying necessary information accurately and asking questions when clarification is needed.
• Establish and maintain effective worki ng relationships with team members, Agency administration, referral sources and community agencies.
• Prepare accurate and timely business correspondence.
• Present accurate information and respond appropriately to, and in a timely manner, questions from managers, clients, client families, and the general public.
• Participate in emergency after hour’s on-call as assigned
• Accurately calculate figures using simple arithmetic.
• Identify and communicate significant changes in client data to appropriate Team members.
• Extensive automobile travel is required and may include exposure to changing weather conditions. Overnight travel may be necessary.
• Home observation responsibilities may include locations not accessible by any other means than walking, some times over uneven terrain and/or climbing stairs.

• Participate in special projects as assigned.
• Participate in/conduct Agency training activities as directed.
