Key Responsibilities
  • To provide an informed and professional response to carers through a confidential service of carer‑centred assessment, support and crisis planning via our Carers Support Line.
  • Provision of advice and information alongside emotional support.
  • Provide short term intervention plans for Carers where appropriate ensuring they are fully supported by effectively managing risk and responsibilities, using a person centred approach.
  • To maintain accurate written and concise computer records in accordance with local policies and data protection and confidentiality legislation.
  • To manage own caseload and have the ability to prioritise tasks and responsibilities whilst ensuring assessments are completed within an agreed timeframe.
  • To work in partnership to improve collaboration and coordination to achieve the best possible outcomes for Carers.
  • To work as part of a team, participating in team meetings, team building, training and activities.
  • To prepare for and participate in regular supervision meetings and to seek additional supervision as required.
  • To demonstrate awareness/understanding of equal opportunities and other people’s behavioural, physical, social and welfare needs.
  • To ensure that reasonable care is taken at all times for the health, safety and welfare of yourself and other persons, and to comply with the policies and procedures relating to health and safety and safeguarding within the organisation.
  • To undertake developmental tasks and carer awareness‑raising as required. This may include being involved with fund‑raising however this will be discussed with the worker on a case‑by‑case basis.
  • To carry out any other duties which fall within the broad spirit, scope and purpose of this job description and which are commensurate with the grade of the post.
What We’re Looking For
  • An understanding of carers needs and issues
  • Previous experience of working in a health care, education, social care or voluntary sector setting including experience of partnership and multi‑agency working
  • Understanding of good practice in information sharing and confidentiality
  • An understanding of safeguarding issues in relation to vulnerable people
  • Understanding of the Care Act
  • Excellent communication skills
  • Ability to work under pressure, prioritise tasks and manage own time and workload effectively as well as working as part of a team
  • A positive attitude to carers and service users
  • Persistence, assertiveness and flexibility
About You

Ideally you will have a relevant Health or Social Care Qualification, but this is not essential.

You will have the ability to actively listen, and empathise with people.

You will have great communication skills both verbally and written.

You have the ability to organise, plan and priority to meet timescales.

You are used to working with IT Software, such as MS Word, Powerpoint, Excel and Teams.

You will have an awareness of safeguarding for vulnerable adults and Health & Safety at all times.

You will have access to a vehicle.

You may already be working within assessment/support planning, whether this is within a GP surgery/health, social care, support work, or other setting.
