£28,007 per annum | 37 hours per week | North Northants (base: Wellingborough) | Fixed term to June 2027 (extension planned to March 2029)

The Role

You'll be on the front line of our Single Point of Access team, carrying out person-centred assessments, developing outcomes-focused support plans, and coordinating wrap-around support for people with complex needs. This isn't a signposting role — you'll see people through every step of their journey.

Key Responsibilities
  • Holistic assessments by phone and in person
  • Developing and delivering individual support plans
  • Coordinating support across statutory and voluntary sector partners
  • Processing referrals and managing enquiries via the inbox and telephone
  • Identifying advocacy needs and safeguarding concerns, taking appropriate action
  • Maintaining accurate case records and data
About You
  • 1–2 years' experience in social care, health, community services or VCSE
  • Experience supporting people with complex or multiple needs
  • Strong relationship-building skills across sectors
  • Committed to EDI and trauma-informed practice
  • Organised, digitally confident, and able to manage a caseload
  • Full driving licence and access to a vehicle (essential)
