Developmental Consultant Liaison

Salary: $40.00 – $50.00

• Conduct assessment screening for newly delivered mothers and babies in assigned hospital location(s) to determine eligibility for Healthy Beginnings referral services.
• Collaborate with assigned hospital personnel to ensure screening assessments are conducted.
• Serve as liaison between hospitals, supervisors, program participants, and collaborating partners.
• Assist eligible program participants to access program services, referrals and linkages.
• Complete all necessary paperwork in a timely manner related to the administration of screens and assessments, partnering agency collaborations, collection of program data, and eligibility requirements.
• Ensure timely input of program data into designated databases to maintain program validity.

• Provide Screening and Assessment services for children through age five (5) and/or their families utilizing approved assessment tools.
• Analyze and interpret screening and assessment data; identify and recommend appropriate services to system participants.
• Effectively engage clients and serve as liaison between program participants and collaborating partners.
• Conduct DC visits with families in their homes or other designated locations within the community.
• Ensure flexibility and accessibility to families by accommodating appointments after school, in the evenings and/or on the weekends when families may be more available.
• Record and report appointments, referrals, home visits, and all other work related Healthy Beginnings Program activities to supervisor on a daily basis.
