We are seeking a dedicated and compassionate General Practitioner to join our healthcare team. The successful candidate will provide comprehensive medical care to patients across a variety of settings, ensuring high standards of patient safety and wellbeing.
To undertake duties governed by the contract and Direct and Local Enhanced Services including:
A variety of duties including face to face surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Main duties of the job- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signsof illness
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes toagreed standards
- Administration - dealing with queries, patient test results,referral letters, task, emails and other paperwork and correspondence in atimely manner
- Compiling and issuing computer-generated acute and repeat prescriptions
- Prescribing in accordance with the practice prescribingformulary (or generically) whenever this is clinically appropriate
- In general the post-holder will be expected to undertake allthe normal duties and responsibilities associated with a GP working within primary care.
- Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate
- Develop care and treatment plans in consultation with patients and in line with current protocols as wellas wider guidance such as that from NICE.
Hatfield PCN is a friendly and supportive PCN. We have
55,000 patients across our 4 practices. There is a wide socio-economic anddemographic served within this area which will provide an interesting andvaried case mix. The role will contribute to improving the quality of patient care across the network of practices.
We are recruiting for 5-7 sessions at Lister House Surgery,Wrafton House Surgery and Spring House Medical Centre. Sessions will be onfixed days in each practice.
This role will work under the clinical supervision of aspecific GP at each practice.
