Pharmacy Technician
We have an opening for a Prescribing Pharmacist in General Practice. They will be responsible for delivering high‑quality, patient‑centred pharmaceutical care within a primary care setting. Working as part of a multidisciplinary team, the pharmacist independently assesses, diagnoses (within competence), prescribes, and manages medications to optimise patient outcomes and ensure safe, cost‑effective prescribing.
The role supports GP workload, improves medicines optimisation, enhances patient safety, and contributes to the overall efficiency and quality of care within the practice.
Main duties of the job- Reconcile medicines following hospital discharge
- Review high‑risk medications (e.g. DOACs, methotrexate, lithium)
- Monitor blood tests and therapeutic drug levels
- Implement local and national prescribing guidance (e.g. National Institute for Health and Care Excellence (NICE))
- Lead audits to improve prescribing safety
- Manage medication queries from patients and community pharmacies
- Improve repeat prescribing systems and processes
- Ensure cost‑effective prescribing in line with formulary
- Conduct prescribing audits
- Support CQC compliance (Care Quality Commission)
- Develop and update practice prescribing policies
- Counsel patients on medication use
- Support adherence
- Provide lifestyle and self‑care advice
- Work closely with GPs, nurses, paramedics, and social prescribers
- Provide medicines advice to the practice team
- Support care homes with medication reviews
- Liaise with community pharmacies and secondary care
We are a large busy practice with a large team of GPs, nurses, ARRs roles, pharmacists and Pharm techs.
We are looking to further enhance our pharmacy team with an additional pharmacist. We are a forward‑thinking and proactive practice who pride ourselves on delivering prompt quality care to our patients.
Job responsibilities- Patient‑Facing Clinical Work
- Conduct structured medication reviews (SMRs)
- Manage long‑term conditions (e.g., hypertension, diabetes, asthma, COPD)
- Independent prescribing within scope of competence
- Manage acute minor illnesses (if qualified)
- Undertake polypharmacy reviews, especially in elderly or frail patients
- Deprescribing where appropriate
- Medicines Optimisation & Safety

- Authorise repeat prescriptions
- Manage medication queries from patients and community pharmacies
- Improve repeat prescribing systems and processes
- Ensure cost‑effective prescribing in line with formulary
- Clinical Governance & Quality Improvement
- Conduct prescribing audits
- Reduce medication errors
- Support CQC compliance (Care Quality Commission)
- Develop and update practice prescribing policies
- Multidisciplinary Team (MDT) Working
- Work closely with GPs, nurses, paramedics, and social prescribers
- Provide medicines advice to the practice team
- Support care homes with medication reviews
- Liaise with community pharmacies and secondary care
- Patient Education & Support
- Counsel patients on medication use
- Support adherence
- Provide lifestyle and self‑care advice
- Manage shared decision‑making discussions
- Masters degree in Pharmacy (MPharm) or equivalent
- Registered Pharmacist with the General Pharmaceutical Council (GPhC)
- Evidence of Continuing Professional Development (CPD)
- Experience in GP practice or Primary Care Network (PCN)
- Experience managing long‑term conditions (e.g., hypertension, asthma, diabetes)
- Experience with electronic prescribing systems (e.g., EMIS, Systm One)
- Experience in audit and quality improvement projects
