Brent Urgent Community Response service is a national leader in intermediate care that aims to bring the best of hospital care to a patient's home and therefore avoid unnecessary hospital attendances/admissions.
Most of our referrals are from GPs, London Ambulance Service and Accident and Emergency departments. Attending in pairs we perform an initial assessment at home, performing a holistic assessment of the patient and in some cases starting a treatment plan within 2 hours of referral.
The team can utilise a number of pieces of diagnostic equipment in the home, plus we have access to hospital facilities for various scans when required. We also perform our services in A&E to avoid hospital admissions and facilitate safe discharges.
Brent Urgent Community response looks to provide the very best patient care, aiming to provide the same care a patient would receive as an in patient. This is facilitated by having real time access to patient records in the community via laptops.
We are looking for practitioners who have a passion for patient care, who want to develop their skills and push themselves outside the normal boundaries of practice and who have a real drive to make sure their patient gets the best treatment possible. Excellent communication skills are essential.
Main duties of the jobAs a Band 6 Nurse you will:
- Attend patients as both part of the Urgent Community Response crew and alone for follow up visits.
- Develop your skills into a more well-rounded holistic approach to patient care.
- Take on extended roles such as venepuncture, drug administration, advance assessment of the acutely ill patient and acute injury assessment.
- Gain experience in generic, orthopaedic, musculoskeletal, respiratory, neurological rehabilitation and falls prevention.
- Gain experience in day to day management of the team including taking handovers and helping to make treatment plans during ward rounds and at the service user home address.
- Supervise junior staff and students
- Attend in-service training and have Continued Professional Development opportunities.

Person Specification Education/Qualification- NMC registered with current valid registration.
- Experience of working in Primary Care/ Accident & Emergency.
- Experience of assessing and providing quality care to people with Long Term Conditions
- Qualification in teaching or practice-based learning.
- Physical Clinical Assessment Skills Course or willingness to undertake the course.
- Substantial band 5 experience
- Experience of supervising staff and appraisals
- Extensive range of clinical skills and experience.
- Experience of working in a multidisciplinary team
- Experience of implementing evidence-based care and developing others to do so
- Ability to manage own workload and plan the workload of others, to work autonomously.
- Working knowledge of clinical governance and evidence-based practice.
- Evidence of comprehensive range of clinical skills and expertise relevant to the role.
- Basic IT skills including use of Microsoft Office package and email.
- Clinical knowledge across a range of long term and enduring conditions.
- Knowledge of current NHS organizational structures and policy initiatives.
- Knowledge of recent developments in Primary Care.
- Knowledge of adult protection and safeguarding children's issues.
- Knowledge of appropriate Code of Professional Conduct.
- Knowledge of Personal Development Planning.
- Detailed knowledge of Primary Health Care and collaborative working with other care providers, e.g. social care, and voluntary services.
- Knowledge of the audit process.
- Knowledge of evidence-based practice and able to implement in practice.
