Summary

The Patient Safety Specialist II working with Director of Risk Management will focus on safety event trends, patient meetings and maintenance of all incidents in the event reporting system (Events/Feedbacks). Ability to have difficult conversations, de-escalate and resolve complex situations with patients. Assists as necessary with department office activities. Ensures that all services are delivered in accordance with the mission statement and values of Confluence Health.

Keeps up to date with trends, initiatives and patient safety activities.

Essential Functions
  • Reviews, enters, edits and closes risk/patient safety and feedback events to ensure they are complete, accurate and triages these events to key stakeholders.
  • Manage patient grievances, including drafting and sending resolution letters in compliance with organizational and CMS standards.
  • Reviews and analyzes patient experience survey results to identify trends and support patient experience improvement initiatives.
  • Schedules meetings, prepares agendas and records minutes as requested for various meetings.
  • Runs reports and creates appropriate graphs as requested by Confluence Health leadership.
  • Processes internal adjustments and external payments on accounts related to quality/risk issues as approved by Patient Advocacy Group (PAG).
  • Monitors accounts on ongoing cases and holds billing as appropriate.
  • Processes compensation payments to patients such as lost belongings.
  • Follows up with patients on determination of their billing complaints/billing issues as discussed at Patient Advocacy Group (PAG) in a friendly and prompt manner.
  • Attends required meetings and serves on committees as requested.
  • Reviews past due reports and works with assigned leadership for timely completion.
  • Conducts trend analysis of feedback and/or safety events.
  • Attends Good Catch/Near Miss event recognition as appropriate.
  • Maintains professional affiliations, attends approved outside conferences, seminars, training, etc.
  • Performs other duties as assigned.
  • Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.

    Qualifications

    Required:

    • Associate's Degree from an accredited college.
    • Minimum of two years working in a healthcare setting.
    • Analytical reasoning skills including statistics, spreadsheet and database skills.
    • Excellent communication and active listening skills.
    • Ability to work with diverse teams representing various disciplines and all levels of staff.
    • Communicates effectively with patients and family members.
    • Proactive, self-motivated and self-directed individual.
    • Excellent multi tasking abilities with ability to manage multiple priorities.
    • Knowledge of Excel, Outlook, Powerpoint.

    Desired:

    • Experience working in an incident reporting system and EPIC.
    • Certified Professional in Healthcare Quality (CPHQ).
    • Certified Professional in Patient Safety (CPPS).
    Physical/Sensory Demands

    O = Occasional, represents 1 to 25% or up to 30 minutes in a 2 hour workday.

    F = Frequent, represents 26 to 50% or up to 1 hour of a 2 hour workday.

    C = Continuous, represents 51% to 100% or up to 2 hours of a 2 hour workday.
