Occupational Therapist – Home Health Case Manager

Location: 11333 W National Ave, West Allis, WI 53227
Job Type: Contract | Standard Rate
Duration: January 12 – April 3, 2026

Pay & Schedule
  • Hours: 40 hours/week
  • Shift: 8-hour day shifts
  • Schedule: Monday–Friday | 8:00 AM – 4:00/4:30 PM

About the Role

Seeking an experienced Occupational Therapist Case Manager to support Home Health services in the North Metro territory. This is a full-time, weekday-only contract with no on-call or weekend requirements—solid structure, consistent workflow, and a traditional home health model that still works.

Job Summary

The OT Case Manager will independently manage a home health caseload, providing occupational therapy services, coordinating patient care, and ensuring accurate documentation and compliance with home health regulations. The role requires strong clinical judgment, organization, and patient-centered care in the home setting.

Key Responsibilities
  • Manage and coordinate a home health OT caseload
  • Perform initial evaluations, reassessments, and discharge planning
  • Develop and implement individualized OT care plans
  • Provide therapeutic interventions focused on ADLs, safety, and functional independence
  • Collaborate with interdisciplinary home health team members
  • Educate patients and caregivers on treatment plans and home safety
  • Ensure timely, accurate clinical documentation
  • Comply with regulatory, quality, and productivity standards
Patient Population
  • Adult and geriatric home health patients
  • Varying diagnoses requiring functional, safety, and independence-focused OT care
Required Qualifications
  • Active Occupational Therapist license (WI or compact, as applicable)
  • Home Health OT experience strongly preferred
  • Ability to work independently in patient homes
  • Strong communication and time-management skills
Floating Requirements
  • Must be willing to float to surrounding territories as needed
Support & Structure
  • Leadership and operational support available
  • Traditional case management model with predictable scheduling
