PURPOSE AND SCOPE:

The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is responsible for providing and coordinating clinically competent care including assessment, planning, intervention, and evaluation for assigned patients. The nurse assesses and manages patients' response to home dialysis training and treatment, following prescribed protocols, and communicates patient issues to the physician as needed.

As part of the End Stage Kidney Disease (ESKD) healthcare team, this role participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

All duties are performed in accordance with Fresenius Kidney Care policies, procedures, standards of nursing practice, and regulations.

  • Perform duties under supervision with guidance from the Educator, Preceptor, or collaborating RN.
  • Collect and analyze dialysis data for assigned patients, document findings, adjust treatment plans, and notify supervisors or physicians as needed.
  • Assess and document patient/family learning needs to provide ongoing education.
  • Coordinate and deliver safe dialysis therapy in collaboration with patients, home care partners, and staff.
  • Administer medications as prescribed, document medical justification and effectiveness.
  • Assist with emergency response measures.
  • Serve as a resource for the healthcare team, participate in staff training and orientation.
  • Ensure proper laboratory procedures are followed, and reschedule missed collections.
  • Update nursing assessments and care plans in collaboration with the interdisciplinary team.
  • Educate patients on transplant options and treatment modalities.
  • Identify, evaluate, and educate Home Dialysis candidates and partners.
  • Assess home environment and partner readiness for dialysis treatments.
  • Train patients and partners on dialysis equipment and supplies through a formal program.
  • Complete CAP requirements for advancement.
  • Perform other duties as assigned by the Supervisor.
  • PHYSICAL DEMANDS AND

    WORKING CONDITIONS:

    This role involves heavy lifting, equipment movement, and patient assistance, with frequent standing and bending. May require moving equipment up to 200 lbs., lifting solutions of up to 30 lbs., and exposure to infectious materials. The work environment is a healthcare facility with typical noise levels. The position involves on-call duties, travel, and may require working in patients' homes.

    SUPERVISION:

    Oversee LPNs/LVNs, RNs, and other staff as a Team Leader or Nurse in charge, after meeting specific experience and training requirements.

    EDUCATION and LICENSURE:

    Graduate from an accredited School of Nursing, with current licensure and BLS/CPR certification, meeting state practice requirements.

    EXPERIENCE AND SKILLS:

    Entry level for RNs with less than 2 years of nephrology nursing experience; preferred 1-year RN experience and home dialysis experience.
