RN Home Health Case Manager – Milwaukee North (West Allis, WI)

If you’re a home-health-savvy RN who’s all about strong assessments, independent workflow, and real continuity of care, this contract is a solid fit. Straight weekday schedule, no call, no weekends—very classic HH setup.

• Monday–Friday
• 8:00 a.m.–4:30 p.m.
• 40 hours/week
• No call
• No weekends
• Holidays as needed
• May float/assist other HH teams

• Minimum 1 year Home Health RN experience
• Must be confident managing:
  • OASIS documentation
  • Start of Care visits (target: ~3 SOCs/week)
  • Patient teaching and independent care planning
• Strong computer documentation skills
• Valid WI or compact RN license
• Active BLS (AHA)

• Wound care: wound vacs, pleural drains, ostomy care
• Peripheral blood draws
• Central line care
• IV administration (elastomeric devices, IV pumps)
• High-level assessment and triage judgment
• Ability to work independently in the field

• Reliable transportation
• Valid driver’s license
• Current auto insurance

• Standard onboarding through the Home Health team
• Additional support available through other teams if floating occurs
