Registered Nurse (RN) – Home Health Case Manager
Position Overview
We are seeking an experienced Registered Nurse (RN) – Home Health Case Manager who is confident working independently in the field and managing a consistent patient caseload. This contract offers a predictable weekday schedule with no on-call or weekend requirements
.
Coverage Area
You will provide care across the following Milwaukee-area ZIP codes
:
53097, 53092, 53223, 53217, 53209, 53224, 53218
Shift & Schedule
- Monday–Friday, 8:00 AM – 4:30 PM
- No on-call
- Holiday coverage as needed
- May float or assist other home health teams
Licensing & Certifications
- Active Registered Nurse (RN) license (WI or compact)
- Active BLS certification (AHA required)
Core Requirements
- Minimum 1 year of recent Home Health RN experience
- Experience managing Start of Care (SOC) visits (goal: 3/week)
- Excellent patient education, assessment, and documentation skills
Clinical Skills Required
- Wound care including wound vacs, pleural drains, and ostomy care
- IV administration (elastomeric and pump-based systems)
Additional Requirements
- Reliable transportation
- Valid driver’s license and active auto insurance
