Rise Well Community Services is seeking a compassionate Full‑Time Social Care Navigator to join our Social Care Network Department. In this role, the Social Care Navigator will be responsible for conducting HRSN screenings, eligibility assessments, and navigation activities for Medicaid Fee‑for‑Service and non‑eligible Managed Medicaid members.

Grant‑funded position – location is remote, however the applicant must be able to report to West Babylon, NY for training and meetings.

• Conduct HRSN screenings.
• Perform eligibility assessments.
• Execute navigation activities for Medicaid Fee‑for‑Service and Managed Medicaid members.

• Bachelor’s Degree preferred.
• Minimum of two years’ experience completing administrative tasks.
• Valid and clean NY State driver’s license.
• Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

$47,500/year (base pay range $47,500.00/yr – $47,500.00/yr).
