Overview

Rise Well Community Services is seeking a compassionate Full-Time Substance Abuse Counselor to join our ACT East Team in Medford, NY. In this role, the Substance Abuse Counselor will provide direct services with a focus on substance abuse recovery and harm reduction on a mobile, multi-disciplinary treatment team for persons with serious mental illness.

Our Assertive Community Treatment teams provide a full suite of treatment to individuals dealing with serious mental illness, to prevent relapse, teach life skills, provide assistance, and connect with essential community resources. As part of the ACT team, you’ll work on a collaborative, mobile, multi-disciplinary treatment team, both in office and in the community.
