Summary:

The case manager is responsible for providing intensive case management to clients with a primary substance use disorder engaged in court diversion programming. The goal is to reduce recidivism and prison populations by diverting non-violent offenders to community-based interventions. Elevate has two positions available in the Treatment Alternative Diversion program.
Location: 1626 Clarence Ct West Bend, WI 53090
Work Week: Monday through Friday, 40 hours
Pay Rate: $24-$26 per hour

Sign-On Bonus Policy:
- 50% paid on second paycheck after start date
- Remaining 50% after 6 months of employment with no disciplinary actions
- If employee leaves before one year, prorated repayment required
Essential Functions:
- Coordinate with Washington County DA’s Office and defense attorneys regarding referrals and eligibility
- Provide case management in areas such as SUD treatment, mental health, education, employment, housing
- Develop case diversion plans, set goals, and monitor client progress
- Support clients in relapse prevention skills and assess their application in real situations
- Collect and document urine drug screens and breath tests
- Coordinate with the Department of Corrections for clients on community supervision
- Maintain accurate documentation and database entries
- Attend court staffing and hearings
Qualifications:
- Bachelor’s in Social Work, Psychology, Health Education, or Criminal Justice
- Licensed Substance Abuse Counselor in Training (SAC-IT)
- Knowledge of confidentiality laws
- Experience in individual/family intervention services
- Excellent communication skills
- Proficient in Microsoft Office and Google apps
- Training in Naloxone administration within 6 months
- Experience in criminal justice/substance abuse preferred
