Washington County located in West Bend, Wisconsin has an excellent opportunity for a Case Manager – Community Treatment Services. This position is responsible for clients diagnosed with mental health and/or substance use involved with outpatient care, targeted case management, Community Support Program, and the Comprehensive Community Services program.
Qualifications- Bachelor’s Degree preferred in social work, human services, or related field.
- One year of supervised work experience.
- Knowledge of local community resources for clients.
- Knowledge of medications preferred.
- A valid Wisconsin driver’s license; available automobile, and automobile insurance required.
- Coordinates with community resources, refers clients for services, and monitors client involvement in services.
- Understands all relevant statutes applicable to the job.
- Participates in administrative and clinical meetings.
- Performs routine clinical assessments, including intake interviews of new referrals.
- Provides support and education to clients, family members, and other natural supports.
- Develops, directs and implements client treatment plans, behavioral approaches, and skill training.
- Serves as an advocate for clients and families.
- Acts as a facilitator or co‑facilitator for individual and group treatment within or outside of the agency.
- Prepares, completes, and submits required items by due dates such as progress notes, service plans, assessments, and court‑required documents.
- Distributes medication to clients in the community when necessary.
- Coordinates with crisis and chapter 51 liaison regarding commitment status and crisis needs.
- Acts as a mandated reporter under current policies and procedures.
- Participates in continuing educational programs.
- Performs other duties as assigned, including responding to an emergency event.
- Knowledge of local community resources available for clients.
- Knowledge of applicable State, Federal and local laws, rules and regulations affecting clients.
- Knowledge of effective treatment protocols.
- Ability to work effectively with a treatment team.
- Effective oral and written communication.
- Interaction with area health care providers, clients and their families.
- Can fulfill physical, sensory (seeing and hearing), and cognitive demands (concentration, conceptualization, memorization).
- A bachelor's degree in social work, psychology, human services, or related field.
- One year of supervised work experience.
- Valid Wisconsin driver’s license.
- Knowledge of medications preferred.
