We are seeking a full‑time Nurse Practitioner to provide psychiatric evaluations, medication management, and follow‑up care for residents in nursing homes and assisted living settings. You’ll be supported by a full‑time medical director, administrative staff, and outsourced billing services—so you can focus on patient care.

• Conduct initial psychiatric assessments and routine follow‑up visits
• Prescribe and manage psychotropic medications
• Collaborate with facility staff, medical directors, and care teams
• Document in EMR in a timely and accurate manner

• Must have a current and active NP license and DEA # in the State of Wisconsin.
• Must have at least one year experience working as a Nurse Practitioner.
• Be willing and able to commute to long‑term care facilities.
• Experience with geriatric populations or interest in working with older adults.
• Compassionate, patient‑centered care philosophy.
• Strong communication and collaboration skills.

Job Type: Full‑time

Pay: Competitive base + productivity bonus (commensurate with experience)

Schedule: Monday to Friday (flexible hours)

Work Setting: On‑site at skilled nursing/long‑term care facilities
