CLTS Case Manager
The Washington County Human Service Department, located in West Bend, Wisconsin, offers an excellent opportunity as a full‑time CLTS Case Manager. The department provides responsive and effective community programs to individuals and families challenged by disability, economic hardship, and safety concerns. Under the general direction of the Youth Treatment Team Supervisor, the Case Manager is responsible for the planning, delivery, and coordination of case management services for children with disabilities and their families.

Under the general direction of the Youth Treatment Team Supervisor, the CLTS Case Manager is responsible for the planning, delivery, and coordination of case management services for children with disabilities and their families.
Qualifications- Bachelor's Degree in a health or human services related field.
- A minimum of two years of professional work experience, preferably working with children with disabilities and their families in community programs.
- Experience in waiver programs preferred.
- Must possess a valid Wisconsin driver's license.
Starting hourly rate is $28.21‑$37.50 based on experience. Washington County offers a comprehensive benefits package:

- Comply with requirements outlined in Medical Assistance (MA) Waiver Manual, Third Party Administration billing, and Children’s Community Options Program.
- Completes assessments as outlined for the Children’s Functional Screen with all required resources information gathered, including annual recertification for the Children’s Long Term Support Programs.
- Develop individual service plans to meet children’s needs defined in outcomes, while complying with the requirements as outlined in MA Waiver Manual and Children’s Community Options Program, ensuring the child’s health and safety with stated outcomes.
- Obtain proposals for services within the guidelines of the CLTS Waiver allowable services, authorize and monitor these services and provider agreements.
- Serve as a liaison between clients, families, service providers, and various state offices.
- Allocate funding based on client needs defined within the Assessment and Individual Service Plan.
- Maintain required contact lines with clients and families as per MA Waiver Manual and Children’s Community Options Program.
- Strategize with families and providers to address issues and concerns, and provide assistance and support.
- Maintain the client’s community living arrangements, preferably within their parental home whenever possible.
- Maintain the case file for each client on the caseload, including meeting time limit standards per the Human Services Department.
- Provide relevant CLTS Waiver and Children’s Community Options Program updates at the quarterly Children’s Advisory Committee meeting.
- Work within a team environment with Human Services staff, community members, school personnel, provider agencies, and DHS staff.
- Perform other duties as assigned, including responding to an emergency event.
