Healthcare PCN, a progressive and forward-thinking network based in Sandwell, is seeking a dedicated Clinical Pharmacist to join our friendly and supportive team. Our PCN currently employs eight Clinical Pharmacists and three Pharmacy Technicians. We aim for an ideal start date in August 2024 but are flexible for the right candidate.

Key Responsibilities
  • Clinical Medicines Review: Provide expertise in clinical medicines review, particularly for high-risk groups like the frail elderly and those with multiple co-morbidities.
  • Medication Management: Oversee the management of medicines for patients discharged from hospitals, ensuring continuity and safety in medication changes.
  • Educational Support: Improve prescribing practices by providing educational support for all prescribers within the practice.
  • Public Health and Social Needs: Address the public health and social needs of patients through comprehensive medication management.
  • Service Improvement: Lead initiatives to enhance clinical care through practice-based audits and adherence to guidelines such as NICE.

Healthcare PCN encompasses six General Practices in Sandwell, covering approximately 42,000 patients. The practices include:

  • Linkway Medical Practice (West Bromwich)
  • New Street Surgery (West Bromwich)
  • Norvic Family Practice (Smethwick)
  • Sarephed Medical Centre (Smethwick)
  • Bearwood Surgery (Smethwick)
Main Duties
  • Poly-Pharmacy Clinics: Run clinics for patients with multi-morbidities to optimize medication.
  • Hospital Discharge Reconciliation: Manage medicines of patients post-discharge without GP referral.
  • Prescribing Practice Improvement: Support and educate prescribers to ensure safe and effective prescribing.
  • Clinical Audits: Develop and manage service improvements through clinical audits.
  • Patient Safety: Ensure safety during transitions between care providers through medication reconciliation.
Analytical and Planning Skills
  • Specialist Clinics: Review patients, optimize their medications, and ensure they receive appropriate care.
  • Cost-Effectiveness: Conduct searches and reviews to improve the practice’s cost-effective prescribing.
  • Coordination of Care: Plan and coordinate the care of patients with specific diseases to meet QOF targets.
Patient Care
  • Holistic Review: See patients in clinics, review their conditions, and optimize medications.
  • Medication Queries: Address patient medication queries remotely or via telephone.
  • Critical Thinking: Utilize advanced critical thinking and decision-making skills in a clinical context.
Policy and Financial Responsibilities
  • Prescribing Initiatives: Implement and evaluate initiatives to improve prescribing practices.
  • Resource Management: Help coordinate and deploy practice resources sustainably.
  • Equipment Care: Maintain a personal duty of care for equipment and resources.
Skills and Qualifications

Essential:

  • Minimum 2 years post-qualification experience
  • Experience with acute and long-term conditions common in general practice
  • Excellent interpersonal and communication skills
  • Proficiency in Microsoft Office packages
  • Registered with the General Pharmaceutical Council (GPhC)

Desirable:

  • Independent Prescribing qualification
  • Experience with Systm One (clinical IT system)
  • Understanding of QOF
  • Membership in relevant professional bodies (e.g., Royal Pharmaceutical Society, PCPA)
Professional Registration and Safeguarding
  • Professional Registration: Must maintain current UK professional registration.
  • Safeguarding: Commitment to safeguarding the welfare of children, young people, and vulnerable adults.
