Responsibilities
- To treat patients in their own place of residence or a clinic setting who have heart failure.
- To co‑ordinate and provide skilled holistic nursing to patients diagnosed with heart failure.
- To assess, plan, implement and evaluate individual programmes of nursing care based on agreed models of care and in consideration of individual need and delegating effectively to others as required.
- Adhere to Trust and National Service Framework and the NICE Guidelines for Heart Failure.
- Providing admission avoidance management and facilitated discharges.
- To be part of the IV furosemide pathway.
- To support junior staff within the team.
- Provide ongoing physical, psychological support, advice and counselling to patients with diagnosed heart failure and their carers as outlined in National Institute for Health and Care Excellence.
- Management of chronic heart failure, in accordance with responsibilities under the NMC Code of Conduct and trust policies.
- The post holder will deliver care within the secondary care setting and provide close links with the acute specialist heart failure and multidisciplinary teams across Primary and Secondary Care.
We are looking for 2 full‑time Band 6 nurses to join our supportive team of nurses and support workers. If you are a Band 6 already or a Band 5 with heart‑failure experience who would like to specialise within a specialist heart‑failure team, then we would like to hear from you.
