Extended Access Practice Nurse + Clinical Lead

The Extended Access Practice Nurse & Clinical Lead is responsible for delivering high-quality, safe, and effective nursing care across the Black Country GP Hub, serving the populations of Citrus Health PCN and Newcomen PCN. The post holder will combine direct patient care responsibilities with clinical leadership duties, supporting the delivery of extended access services across evenings, weekends, and bank holidays. As the Clinical Lead Nurse, the post holder will be the first point of contact for Practice Nurses, Advanced Nurse Practitioners (ANPs), and Healthcare Assistants (HCAs) working within the Extended Access Service.

They will work closely with the Hub Management Team, GP Clinical Leads, and PCN leadership teams to ensure contractual requirements, quality standards, governance arrangements, and patient outcomes are consistently achieved. The role requires a highly motivated, experienced Primary Care Nurse with strong clinical, leadership, and organisational skills who is passionate about delivering excellent patient care and supporting workforce development.

Clinical Practice

The post holder will:

  • Assess, plan, implement and evaluate programmes of care to promote health and wellbeing.
  • Deliver evidence-based nursing interventions in line with national and local guidance.
  • Manage and review patients with long-term conditions, supporting self-management and personalised care planning.
  • Identify patients at risk of developing long-term conditions and initiate appropriate interventions.
  • Provide health promotion, screening and preventative care to improve patient outcomes.
  • Undertake opportunistic health promotion during patient consultations.
  • Deliver cervical cytology screening and women's health services.
  • Offer family planning advice and support.
  • Assess, manage and treat uncomplicated wounds.
  • Deliver childhood and adult immunisation programmes.
  • Provide travel health advice and vaccinations where appropriate.
  • Recognise, assess and refer patients presenting with mental health concerns.
  • Provide advice regarding prescribed and over-the-counter medications.
  • Maintain accurate, contemporaneous, and legally compliant clinical records.
  • Work within professional competence and NMC standards at all times.
Clinical Leadership

The post holder will:

  • Provide visible clinical leadership and professional support to Practice Nurses, ANPs, and HCAs working within the Extended Access Service.
  • Act as the first point of escalation for clinical and operational nursing queries.
  • Lead by example, promoting high standards of patient-centred care.
  • Support workforce planning and service development initiatives.
  • Ensure nursing staff work within professional standards, clinical policies and evidence‑based practice.
  • Promote a culture of continuous quality improvement and shared learning.
  • Contribute to service planning and the achievement of contractual performance targets.
  • Support recruitment, induction, supervision and retention of nursing staff.
  • Facilitate team development and encourage collaborative multidisciplinary working.
Clinical Governance & Quality

The post holder will:

  • Ensure all clinical activity is delivered in accordance with NMC standards, NICE guidance, local policies, and national frameworks.
  • Maintain awareness of regulatory updates and implement changes where required.
  • Review and update clinical protocols and operating procedures.
  • Participate in audits and quality improvement initiatives.
  • Monitor service performance and support action planning where improvements are identified.
  • Contribute to the investigation of complaints, incidents, significant events and near misses.
  • Undertake root cause analysis and support implementation of learning outcomes.
  • Promote patient safety, clinical effectiveness, and continuous quality improvement.
  • Support safeguarding processes for children and vulnerable adults.
  • Work within organisational policies relating to domestic abuse, substance misuse, and vulnerable groups.
