Part-time Post-Acute Care Opening in West Chester, PAPhiladelphia 35mSeeking a part-time primary care physician to manage complex patient cases in a post-acute, skilled nursing facility setting in West Chester, PA. Part-time (2 days/week) Day call M-F, Day call weekend rotationDuties Primary care rounding in skilled nursing facilities Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Collaborate with clinicians, facility leaders and others regarding care and services Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery Provide oversight, guidance, and mentorship to our nurse practitioners Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issuesQualifications Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered care Post-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunities Active and unrestricted Pennsylvania license (or in process) Active and unrestricted DEA/prescriptive authority (or in process) A deep commitment to improving patient outcomes, leading clinical excellence, and driving change in the healthcare landscape Internal medicine (IM), family medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are welcome to applyCompensation Compensation is fee for service (FFS).
