Position: REGISTERED NURSE FULL-TIME DAY- W. Covina, CA

Registered Nurse Full‑Time Day – Covina, CA

Employer: Kindred / Scion Health

Location: West Covina, CA

Pay Range: $42.80 – $53.22 per hour

Sign‑On Bonus: $15,000.00 (requires one‑year commitment)

Job Summary: Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with physicians, nursing supervisor/manager, and co‑workers as appropriate. Participates in discharge planning process.

Essential Functions
  • Maintains nursing care standards and implements hospital policies and procedures.
  • Directs, supervises, provides and evaluates nursing care provided to patients.
  • Assigns or delegates tasks based on patient needs, potential for harm, task complexity and staff scope of practice.
  • Documents patient admission assessment, reassessments, care plans and other pertinent information in the patient’s medical record.
  • Performs assessments on all patients upon admission and at reassessment intervals; makes referrals to other disciplines as appropriate.
  • Develops and updates nursing care plans; ensures coordination with patient, family, and team members.
  • Assesses and manages pain, providing education to patients and families.
  • Revises plans of care based on patient response and evaluates effectiveness daily.
  • Executes patient care responsibilities in accordance with the standard of care for patient age.
  • Receives physician orders, ensures accurate transcription and documentation.
  • Administers medication using the five rights of medication administration.
  • Formulates teaching plans based on identified learning needs; evaluates effectiveness.
  • Assists physicians with examinations, treatments and procedures; performs service within scope of licensure.
  • Provides respectful, dignified care and addresses psychosocial, cultural and spiritual needs.
  • Interprets patient status data to identify age‑specific needs.
  • Provides care in a patient‑safety‑oriented environment.
  • Initiates or assists with emergency measures for adverse patient conditions.
  • Responds to calls, paging, patient requests and rounds efficiently.
  • Consults other departments for interdisciplinary care provision.
  • Completes end‑of‑shift report; ensures narcotic count, documentation, and order sign‑off.
  • Communicates clearly with management, co‑workers and physicians.
  • Collaborates with Social Services/Discharge Planners for inpatient and post‑discharge needs.
  • Participates in orientation, instruction and training of new personnel.
  • Safely manages and operates equipment.
Knowledge / Skills / Abilities
  • Knowledge of medications and correct administration based on patient age and clinical condition.
  • Basic computer proficiency.
  • Organizes tasks, sets priorities and functions under stressful conditions.
  • Maintains good working relationships across departments.
  • Must read, write and speak fluent English.
  • Maintains good attendance.
  • May perform other related duties as assigned.
Qualifications

Education: Graduation from an accredited Bachelor of Science in Nursing, or an Associate Degree in Nursing or Nursing Diploma program.

Licenses / Certification:

  • Current state licensure as Registered Nurse.
  • BCLS certification required.
  • ACLS certification preferred.

Experience: Minimum six months of Medical/Surgical experience in an acute‑care setting preferred.
