Responsibilities
  • Provide care and services for a group of patients visiting their residences, which could include private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Serve as a contributing member of the interdisciplinary team, offering expertise on psychosocial assessments, interventions, and knowledge of community resources.
  • Work in either a Case Management, Admissions, or Resource role.
Qualifications
  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Ability to work 1-on-1 with patients while receiving strong support from a patient care team.
  • Ability to work independently with day-to-day activities including phone, email, written communication, meetings with healthcare professionals, and understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings, prioritizing and triaging multiple concerns.
  • Ability to learn an electronic medical records system.
  • Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to travel across the service area.
