As a Home Health Wound Care Nurse, you will provide specialized wound assessments, treatments, and ongoing clinical oversight while collaborating with physicians, patients, families, and the interdisciplinary care team. ** Essential Responsibilities
*** Perform comprehensive nursing assessments and develop individualized plans of care.
* Assess, monitor, and treat a variety of wounds, including surgical wounds, pressure injuries, diabetic ulcers, vascular wounds, and traumatic injuries.
* Perform wound measurements, staging, documentation, and ongoing evaluation of healing progress.
* Administer wound care treatments in accordance with physician orders and evidence-based practices.
* Educate patients, families, and caregivers regarding wound care, prevention strategies, nutrition, and disease management.
* Monitor for signs and symptoms of infection or complications and communicate changes in condition to the physician and care team.
* Coordinate care with physicians, therapists, aides, and other healthcare professionals.
* Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
* Ensure accurate, timely, and compliant documentation within the electronic medical record.
* Participate in case conferences and interdisciplinary team meetings.
* Maintain compliance with all federal, state, and agency regulations.

** Qualifications
* ** Current Registered Nurse (RN) license in the State of Iowa.
* Minimum one year of nursing experience required.
* Home health and/or wound care experience preferred.
* Strong assessment, critical thinking, and clinical documentation skills.
* Ability to work independently and manage a flexible schedule.
* Valid driver's license, reliable transportation, and current auto insurance.
* Current CPR certification.
