Overview

Residential Aide - Per Diem - Farmington at Continuum of Care

Performs direct care and supports individuals with daily living activities, builds skills, and fosters engagement in group activities. Works with licensed or visiting nursing personnel to monitor medical concerns and medications, report issues, and ensure safe, sanitary, and well-organized living spaces. Leads or co-leads group activities and outings, collaborates with nursing, management, and coworkers to deliver high-quality services, and performs other duties as assigned.

job functions
  • Client Care:
    Establishes rapport and professional boundaries; enhances independent living skills by engaging, assessing, and providing teaching and assistance (e.g., modeling, prompting, hands-on support).
  • Supports activities of daily living (ADLs) such as personal care/hygiene, housekeeping, laundry, food management/nutrition, budgeting, health and safety, and leisure activities to promote healthy living.
  • Facilitates psycho educational groups and participates in community integration and social recreation activities.
  • Provides transportation to and from appointments and educates clients on travel planning and use of transportation.
  • Keeps living spaces clean; conducts apartment/room inspections and assists with maintaining cleanliness; conducts rounds to ensure safety.
  • Monitors safety and responds to crisis situations following agency policy for Nonviolent Practices.
  • Supervises and documents self-administration of client medications as applicable.
  • Attends monthly staff meetings, clinical rounds, All Treater meetings, and supervision sessions.
Environmental responsibilities
  • Maintains a clean, well-organized work environment; ensures exits and walkways are clear for safety.
  • Notifies supervisor of maintenance and safety issues and generates work orders as needed.
  • Participates in fire and emergency evacuation drills and reviews the emergency preparedness plan.
  • Participates in Food Bank activities as assigned; staff may be required to remain awake during all shifts for security and safety.
Quality assurance
  • Adheres to policies and practices from the staff handbook and program operations manual; demonstrates respectful, constructive, and cooperative work conduct.
  • Upholds HIPAA regulations and secures necessary releases of information for outside communication.
  • Models culturally competent behavior; maintains accurate electronic records and case files in accordance with programming, contractual, licensing, and accreditation requirements (e.g., CARF).
  • Participates in agency committees when available; completes trainings as required and attends in-services, trainings, and meetings to support professional growth and quality of case management practices.
Qualifications
  • High School Diploma or equivalent; minimum of one year of experience in a related healthcare field. Licensed CNA or other related healthcare certification may be accepted in lieu of experience.
  • Prior experience working with individuals with severe and persistent mental illness, intellectual disabilities, or addiction; knowledge of community support systems and resources preferred.
  • Valid Connecticut Driver’s license with a clean driving record; for scattered-site/non-24 hr programs, ability to transport clients in personal vehicles may be required.
  • CPR/First Aid certification offered during orientation; maintain ongoing certification.
  • Proficient computer skills, including Microsoft Office and electronic record keeping.
