Responsibilities
  • Primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population.
  • Evaluate and treat in the most functional environment which is the patient’s home.
  • Assess need, develop and deliver physical therapy programs to facilitate rehabilitation.
  • Make referrals to other disciplines to address comprehensive needs of patients.
  • Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
  • Instruct, educate and train patients and caregivers in essential skills and functions to promote independence and productivity.
  • Direct activities to promote and maintain health.
  • Supervise PT Assistants, temporary licensees, students and support personnel as required.
  • Adhere to department and patient treatment schedule, timely clinical and billing documentation requirements.
  • Attend and contribute to patient care, staffing conferences and related meetings.
  • Consult with other rehabilitation team members to select appropriate therapeutic regimen.
  • Adapt program and/or plan of care according to the needs of the individual patient.
  • Promote a safe environment and enforce elimination of fire and safety hazards.
  • Order supplies and equipment as necessary; clean, maintain and report needed repairs.
  • Screen, examine and evaluate patients, establishing diagnosis, impairments, predicted improvement and time required.
  • Develop appropriate treatment goals and methods; implement physical therapy treatment plan; complete related documentation.
  • Provide therapeutic techniques such as exercise, functional training, manual therapy, airway clearance, integumentary repair, modalities, health and wellness, education on chronic disease management, falls assessment, case management.
  • Design, fabricate, fit and/or apply adaptive devices and train patients in the use of adaptive devices.
  • Instruct patient in mobility activities and evaluate progress.
  • Perform routine reexamination and modify/progress plan of treatment.
  • Incorporate health and wellness programming into treatment plans for patients.
  • Perform home and community mobility assessments and recommend safe mobility in home and community space.
  • Perform other related duties as required.
Qualifications
  • Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
  • Master’s degree in Physical Therapy.
  • Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy.
  • Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
  • Physical Therapy degree earned outside of the U.S. evaluated to be the U.S. equivalent to a degree in Physical Therapy.
  • Valid driver’s license or reliable, independent transportation to and from the patients’ residence.
Posted Salary Range

USD $65.00 - USD $66.00 /Hr.
