CERTIFICATION & LICENSURE:

• LVN/LPN of California
• Active CA license
• 1 year of case management experience in acute or outpatient setting
• Knowledge of HMO and Medicare for the inpatient, home health, and SNF settings
• Travel Experience Required

PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

• Minimum of 2 years of acute care experience
