The Physical Therapist is responsible for delivering direct care to patients and utilizing physical agents as prescribed by a referring physician.

• Evaluates patient and creates treatment plan
• Employee is able to provide verbal and written educational material to patients when appropriate
• Recognizes the need for involvement of other disciplines
• Creates a recommended discharge plan
• Performs and records daily clinical notes
• Participates in performance improvement initiatives and completes department mandatory in‑service/training/competency within specified time frame
• Participates in education of staff and students within the medical center

• Bachelor of Science Degree in Physical Therapy or Master’s degree or higher in Physical Therapy. Doctor of Physical Therapy (DPT) preferred.
• Licensed and registered PT in the State of New York.
• BLS required.

USD $60.94 - USD $60.94 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
