Job Responsibilities- Answers incoming telephone calls to the department. Takes a complete message and forwards call or message to appropriate staff.
- Assists with concrete transition planning, including ordering supplies, transportation arrangements, DME, faxing/emailing related correspondence to facilities and agencies.
- Assists with obtaining authorization for post‑acute services when required.
- Assembles and facilitates paperwork for QIO expedited appeals.
- Maintains department files (paper and electronic).
- Responsible for sending and receiving department faxes.
- Distributes census, reports required for Care coordination staff.
- Orders supplies/equipment, oversees office needs and performs various duties as required.
- Performs duties as required to support the daily functions of the Care coordination Department.
- Manages the Hub Support team’s productivity and performance.
- Trains new team members and provides necessary coaching if needed.
- Escalates insurance authorization delays to payer leadership and/or hospital leadership.

- Proficient in the use of a personal computer – including standard office software Word, Excel; experience with basic file and directory management required; experience with spreadsheet and database software and/or mainframe systems preferred.
- Proficient in the use of office equipment such as faxes, copiers, scanners.
- Demonstrated ability to work with minimal supervision once instructed and proficient in required activities.
- Ability to effectively communicate in a verbal and written manner; demonstrate ability to be organized, neat and detail oriented.
- Demonstrates a courteous and professional demeanor, team spirit and the ability to work with office staff and all hospital personnel in a collaborative manner.
- Knowledge of medical terminology preferred.
- High School degree required. Two years college experience or associate’s degree preferred.
- Hospital or medical office experience preferred.
