We are seeking an experienced Clinical Pharmacist who is an Independent Prescriber to support a service in Preston. This position requires an individual who is confident working autonomously and has current clearances related to drug misuse services and secure environments. The successful candidate will play a key role in delivering high quality medicines management, prescribing support and patient‑centred care within a busy clinical setting.

• Provide clinical pharmacist support including medication reviews and medicines optimisation.
• Prescribe safely and appropriately as an Independent Prescriber.
• Manage patients with long term conditions and complex medication regimes.
• Work closely with multidisciplinary teams to ensure effective clinical pathways.
• Maintain accurate prescribing and clinical documentation.

• Fully qualified Clinical Pharmacist.
• Independent Prescriber (IP) qualification.
• Drug misuse clearance.
• Prison clearance or eligibility to obtain it.
• Strong clinical decision making and communication skills.
