Home Health Registered Nurse – Part Time, Fayette County

Join UPMC Home Health as a Part‑Time RN covering Fayette County and part of Westmoreland County. This daylight‑hour position (8:00 am–4:30 pm) includes some weekend assignments. You will provide direct, one‑on‑one patient care in home settings, collaborating with a multi‑disciplinary team to deliver personalized care.

• Develop patients’ plans of care in collaboration with physicians, patients, and utilization management professionals.
• Coordinate case management across the multi‑disciplinary health care team.

• Complete required documentation (visit assessments, OASIS, 485, verbal orders) accurately and promptly.

• Graduate of an accredited professional nursing school is required; bachelor’s degree preferred.
• Prior nursing experience preferred.

• Current RN license in the state of employment or a state covered by the Nursing Licensure Compact.
• CPR certification per AHA standards, both didactic and skills demonstration within 30 days of hire.
• Valid driver’s license and automotive insurance.
• Compliance with Act 33, Act 34, OAPSA, and other applicable local regulations.
• For out‑of‑state licensing under the NLC, employees with residency changes must apply within 60 days.
