PRN Occupational Therapist

Functional Pathways (Daughters of Israel) – West Orange, NJ

Job Summary

The Occupational Therapist shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the Physician.

Responsibilities
  • Screen all residents in the facility on a quarterly basis to identify those with functional declines and determine if OT intervention is indicated.
  • Evaluate residents per MD order and establish appropriate treatment plans and goals that meet the specific needs of the individual.
  • Reassess patients and caseload every 30 days to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
  • Supervise treatment implementation carried out by Certified Occupational Therapy Assistants and determine appropriateness, as well modify treatment plan as indicated.
  • Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
  • Educate staff, family, and care givers as indicated.
  • Identify needs for program development and implement programs according to company policies and procedures.
  • Identify equipment needs of Occupational Therapy Dept. and order equipment according to company and facility policy.
  • Complete all required documentation according to guidelines specified in Documentation Manual.
  • Coordinate in-services for facility staff as needed, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall call of the residents.
  • Attend all facility required meetings.
  • Promote profession within facility, including coordinating special events during National Occupational Therapy Month.
  • Attend Continuing Education courses specific to current position.
  • Maintain professionalism and always follow all national/state association’s regulations.
  • Supervise student affiliates as directed.
Qualifications
  • Bachelor's, Master’s or Doctorate degree in occupational therapy from an accredited program.
  • Certification and licensure requirements as established by state and national board associations.
  • Active state license.
  • Excellent organizational and time management skills.
  • Excellent written and verbal communication skills.
  • Current working knowledge of Functional Pathways policies and procedures.
  • Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
