LPN Emergent Field Visits - Nights - Delray​/West Palm​/Jupiter

Join us for the LPN Emergent Field Visits - Nights - Delray Beach/West Palm Beach/Jupiter role at Trustbridge.

Pay range: $20/hr.

- $27/hr., based on experience

Use your nursing skills to make a difference. Hospice nurses provide end-of-life care to ensure comfort and quality of life. They work in collaborative environments requiring communication with patients, families, and caregivers. Ideal candidates are detail-oriented, patient, calm, caring, and kind, with a hospice heart to help patients celebrate their life’s journey.

• Hours:
  
  Sat/Sun and one weeknight, 7 pm - 7:30 am
• Identify patient/family needs and coordinate care under RN and physician supervision
• Contribute to and follow individualized care plans within the interdisciplinary team
• Utilize monitoring, intervention, and teaching skills to maximize comfort and quality of life
• Prioritize patient and family comfort and care
• Provide personalized, sincere, and engaging service to patients and families

• Current LPN license in Florida
• Basic Life Support Certification
• Completion of 30-hour IV Course per Florida Board of Nursing
• Hospice Certification preferred
• Understanding of hospice nursing, case management, crisis intervention, communication, thanatology, and needs assessment
• Reliable transportation, valid Florida driver’s license, and insurance
• Ability to coordinate with volunteers and healthcare professionals
• Skills demonstrated through training and RN supervision
• Bilingual (English/Spanish) preferred
