Job Summary: We’re looking for a dependable and compassionate Licensed Practical Nurse (LPN) to join our home health team. In this role, you’ll provide skilled nursing care in patients’ homes, following an established plan of care and working closely with RNs, therapists, and caregivers. You’ll need strong clinical judgment, attention to detail, and excellent communication skills to advocate for your patients and support their healing ignments would be in North Palm Beach County, Belle Glade, and South Palm Beach County areas.

Responsibilities:

• Provide direct nursing care (including wound care, injections, and medication administration) as outlined in the patient’s care plan
• Monitor patients’ condition and report any significant changes to the supervising RN or physician
• Accurately document each visit, treatments provided, and patient responses in compliance with agency and regulatory guidelines
• Educate patients and caregivers on proper care techniques and health maintenance
• Communicate and coordinate with care team members to ensure the best outcomes for the patient

Qualifications:

• Active LPN license in the state of Florida (or applicable state)
• Home health experience preferred
• IV certification preferred
• Current CPR/First Aid certification
• Reliable transportation and a valid driver’s license
• Strong observation, time management, and interpersonal skills
