Wound Care Nurse LPN​/RN

Position: Wound Care Nurse
Facility: Beach Breeze Rehab and Care
Location: Boca Raton, Florida
Shift: Full-Time 7am-3pm
Pay Rate: RN up to $40/hr | LPN up to $35/hr (based on experience)

Beach Breeze Rehab and Care is seeking a skilled and compassionate Wound Care Nurse to join our dedicated clinical team. In this essential role, you will work closely with the Interdisciplinary Team (IDT) to provide exceptional wound care, support healing, and enhance overall resident well‑being.

• Collaborate with the IDT to plan, implement, and evaluate individualized resident care plans.

• Assist with completing relevant sections of the Minimum Data Set (MDS) as needed.

• Review, update, and maintain skin and wound care policies and procedures.

• Report any suspicion or knowledge of fraud, abuse, waste, or criminal activity to the Administrator or designee.

• Assess, manage, and treat a range of wounds including pressure injuries, ostomies, surgical incisions, traumatic wounds, and burns.

• Identify and minimize resident‑specific risk factors for pressure ulcers and wound infections.

• Support pain management related to wounds and conduct ongoing skin assessments.

• Educate residents and families on treatment plans and wound care practices.

• Evaluate residents’ nutritional needs to optimize wound healing.

• Recommend pressure‑relieving devices, therapies, dressings, and wound care products to the IDT.

• Current, active, and unencumbered RN or LPN license in the state of Florida.

• Minimum of two (2) years of clinical nursing experience (RN or LPN).

• Strong understanding of wound management, pressure ulcer prevention, and interdisciplinary teamwork.
