Trustbridge Hospice, a member of Empath Health, is seeking a compassionate Registered Nurse to join our Admissions Team serving Palm Beach County. The RN will evaluate referred patients, educate families on hospice care, and coordinate individualized care plans with physicians and facilities in a Friday–Sunday 9:00–21:30 schedule.
Ideal candidates hold a Florida RN license (BSN preferred), hospice or home health experience, and are proficient with EMR systems.
